Insurance
Please bring your insurance card to every visit. Please be aware that your insurance may only partially cover services provided and coverage varies among insurance carriers, plans and employers. It is your responsibility to notify us of any changes in or termination of your health insurance coverage. We know that health insurance coverages may be complex and difficult to understand. We encourage you to discuss questions with your insurance carrier in order to receive the maximum benefits available under your plan. Since our relationship is with you and not your carrier, the fiancial responsibility for payment lies with you. Payment for balances remaining once insurance clears is due upon receipt of the first statment.
